Spay/Neuter Agreement 

Permanent ownership of this dog is contingent upon your compliance with this Addendum. Proof of the spay/neuter must be received by BEAVER CREEK KENNELS within fourteen (14) days after the surgery. In the event that the dog's health does not allow this agreement to be honored, BEAVER CREEK KENNELS must be provided with a statement from your veterinarian that this dog is not yet in physical or emotional condition for surgery. Based upon the veterinarian's evaluation of this dog the BUYER will contact the veterinarian and establish the earliest date that the procedure can be performed. The BUYER will notify BEAVER CREEK KENNELS of this amendment in writing, at which time all other conditions of this agreement will apply and be enforced. This is the responsibility of the BUYER, not the veterinarian, to ensure that BEAVER CREEK KENNELS has received verification that the surgery has been performed. A copy of the medical record as well as the receipt is necessary verification along with the veterinarian’s phone number to follow up about the procedure. Failure to comply with this agreement by the date provided, unless otherwise agreed to in writing, will be considered a breach of the Contract, and BUYER will transport this dog back to BEAVER CREEK KENNELS and will NOT be entitled to a refund. BEAVER CREEK KENNELS also requires the BUYER to pay a $100 fee at the time of pick up of said puppy which will be refunded once BUYER provides proof that said puppy has been spayed / neutered which must be done before the age of one years old. In the case of the puppy's death on or before the above listed date, a written statement from your veterinarian must be sent to BEAVER CREEK KENNELS, stating that the puppy has indeed passed away, and the cause of death. If BEAVER CREEK KENNELS is notified that said puppy has been bred the BUYER will be fined $500 and the BUYER will transport this dog back to BEAVER CREEK KENNELS and will NOT be entitled to a refund. This Includes the $100 fee that is held until proof of spay / neuter.  

In a perfect world, we would like to have each puppy we place to stay in their original purchasing home. Things do happen, and should you need to transfer ownership before spay/neuter has been performed, you agree to contact BEAVER CREEK KENNELS, in writing, phone or email and let us know BEFORE the puppy has been placed into a new home. We require a right of first return or reclamation, that the puppy be returned to us if you cannot keep him or her. You are not free to give the puppy away to another family member or anyone without receiving permission from us first. If you decide to go ahead and give the puppy to someone without contacting BEAVER CREEK KENNELS, this contract is still your responsibility to enforce in the puppy's new home. Should we not receive confirmation of the puppy's spaying or neutering by the above listed age, as the original purchaser, you will then be liable for a fine of $500. 

By my signature below, I agree to have this dog spayed/neutered by the age of ONE YEAR (12 MONTHS) and I understand that this spay/neuter agreement is an agreement that this dog will not produce a litter of puppies either as purposely bred or by accidental breeding. I also agree that if said puppy is NOT SPAYED/NEUTERED. I will return said puppy back to the seller/breeder with no refund. And have a $500 fine for not spaying / neutering. I also realize that there is a $500 fine if said dog is bred and I will not receive my $100 fee that was paid when we picked up said puppy If said puppy is bred or not spayed/neutered. 

This is an entire agreement (3 pages) between the parties and is valid and agreed to by all parties. All parties have read and understood and agreed to this contract in full by signing below.
